In this vital role, you will provide comprehensive support to clients in their homes, ensuring their safety, comfort, and well-being. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those managing chronic conditions such as dementia or autism. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance Monitor patient health by checking vital signs, observing changes in condition, and reporting findings accurately Administer medications following care plans while adhering to HIPAA regulations for patient privacy and confidentiality Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Support patients with memory care needs such as Alzheimer's or dementia through specialized techniques and patience Operate assistive devices like Hoyer lifts or wheelchairs safely during transfers and mobility support Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with healthcare standards Perform light cleaning duties to ensure a safe and hygienic environment within the home or community setting Collaborate with healthcare professionals to develop or update personalized care plans based on evolving needs Provide emotional support and companionship to foster social engagement and reduce feelings of isolation Experience Prior experience working in home health, assisted living facilities, nursing homes, or group homes is highly preferred Demonstrated knowledge of patient observation techniques and vital signs monitoring Hands-on experience with medication administration and understanding of care plans is essential Familiarity with working with individuals who have developmental disabilities, autism spectrum disorder, or dementia enhances effectiveness in this role Certification in CPR and First Aid is required; additional certifications such as HIPAA training are advantageous Experience using mobility aids like Hoyer lifts or handling residents' (personal belongings) respectfully is beneficial Ability to lift heavy objects safely when assisting with transfers or mobility support is necessary Knowledge of hospice care principles and home & community care practices supports comprehensive client support Strong social work skills help address emotional needs alongside physical care requirements This role is essential for delivering high-quality, compassionate care that promotes independence while ensuring safety.
