We are looking for a compassionate, organized, and proactive Care Coordinator to join our growing home care team. About Us At ComForCare Home Care, we believe home care is more than staffing shifts — it is about building trust, supporting families, and helping seniors age safely in the comfort of their homes. We specialize in dementia care, Parkinson's support, fall prevention, and aging in place. Responsibilities Serve as a primary point of contact for clients, families, and caregivers Coordinate start-of-care services and onboarding of new clients Conduct regular client and caregiver check-in calls Help match caregivers with clients based on personality and care needs Monitor client satisfaction and address concerns proactively Support scheduling team during urgent staffing situations or call-offs Communicate changes in client conditions to appropriate team members Maintain accurate documentation and client records Build strong relationships with caregivers, referral partners, and families Assist with care plan updates and service coordination Help ensure high-quality customer service and caregiver accountability Qualifications Experience in home care, healthcare, customer service, case management, or caregiving preferred Excellent communication and relationship-building skills Strong organizational and multitasking abilities Compassionate, patient, and solution-oriented mindset Comfortable handling sensitive situations and escalations professionally Ability to work in a fast-paced environment Experience with scheduling software or home care systems is a plus Knowledge of dementia care, Parkinson's, or senior care preferred What We're Looking For We are looking for someone who: Truly cares about seniors and families Communicates clearly and professionally Takes initiative and follows through Can problem-solve calmly under pressure Builds trust with both clients and caregivers Understands that customer satisfaction starts with communication Benefits Meaningful and rewarding work Growth opportunities within the company Supportive team environment Competitive pay based on experience Opportunity to make a real difference in the community To apply, call: 512-957-0006 Send resume and brief introduction.
