Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or transfer belts Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately while adhering to HIPAA privacy regulations Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Administer medications following prescribed care plans and document medication administration in compliance with healthcare standards Support clients with cognitive impairments such as dementia or Alzheimer's disease through memory care techniques and engagement activities Conduct regular patient observation to identify potential safety concerns or health issues promptly Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed Requirements Proven experience in home health, assisted living, nursing home settings, or working with individuals with developmental disabilities or autism spectrum disorders Certification in First Aid and CPR; additional training in hospice or dementia care is a plus Knowledge of HIPAA regulations to ensure client confidentiality and privacy at all times Ability to lift heavy objects safely and assist with patient transfers using appropriate equipment such as Hoyer lifts or resident lifts Strong observational skills for monitoring patient condition and recognizing signs requiring medical attention Experience working with care plans, medication administration, and vital signs measurement Compassionate attitude combined with excellent communication skills to build trusting relationships with clients and their families This role offers an exciting opportunity to provide essential support that improves lives.
