Intake Care Coordinator Position Summary:
The Intake Care Coordinator serves as the first point of contact for prospective clients, families, referral sources, and healthcare professionals seeking in-home care services. This position is responsible for coordinating the intake process, conducting initial client assessments, scheduling consultations, verifying service eligibility, and ensuring a smooth transition into care. The Intake Care Coordinator works closely with clients, caregivers, and office staff to match clients with appropriate caregivers and maintain exceptional customer service. Key Responsibilities Receive and respond to inquiries from prospective clients, family members, hospitals, rehabilitation facilities, physicians, and community partners. Conduct telephone and in-person intake assessments to identify client needs, preferences, and care requirements. Explain available in-home care services, pricing, and service options to prospective clients and families. Coordinate and schedule in-home assessments and start-of-care visits. Collaborate with scheduling staff to match clients with qualified caregivers based on care needs, personality, and availability. Obtain and maintain required intake documentation, service agreements, and client records. Verify insurance, long-term care benefits, veterans' benefits, or other funding sources when applicable. Maintain regular communication with referral sources and community partners. Follow up with prospective clients and families regarding service inquiries and admissions. Monitor client satisfaction during the onboarding process and address concerns promptly. Ensure compliance with company policies, state regulations, HIPAA, and confidentiality standards. Maintain accurate documentation in the agency's client management system. Qualifications High school diploma or GED required; Associate's or Bachelor's degree preferred. Minimum of 1-2 years of experience in home care, healthcare intake, customer service, or care coordination. Knowledge of non-medical home care services and senior care preferred. Strong computer skills, including Microsoft Office and scheduling software. Excellent verbal and written communication skills. Ability to work in a fast-paced environment and manage multiple priorities. Required Skills Exceptional customer service and relationship-building abilities. Strong organizational and time-management skills. Ability to handle sensitive and confidential information professionally. Problem-solving and critical-thinking skills. Ability to work independently and collaboratively with office staff and caregivers. Strong attention to detail and follow-through. Working Conditions Office-based position with occasional community outreach and client home visits. May require occasional evening or weekend availability to support client admissions and urgent care needs.
Reports To Director of Operations, Client Services Manager, or Branch Manager Primary Goal:
To provide a seamless, compassionate intake experience that connects clients and families with quality in-home care services while supporting agency growth and client satisfaction.
Pay:
$18.00 - $27.32 per hour
