In this vital role, you will provide comprehensive support to promote independence, safety, and well-being in a home or community setting. Your dedication will help clients maintain their quality of life through attentive caregiving, health monitoring, and compassionate companionship. This paid position offers an opportunity to work closely with individuals with diverse needs, including seniors, those with disabilities, or individuals recovering from illness or injury. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts. Monitor patients' vital signs and observe for any changes in health status, promptly reporting findings to healthcare professionals. Administer medications accurately following care plans while adhering to HIPAA regulations to ensure privacy and confidentiality. Prepare nutritious meals tailored to dietary restrictions and preferences, ensuring safety and proper nutrition. Maintain detailed documentation of care provided, patient observations, medication administration, and any incidents in compliance with healthcare standards. Support clients' emotional well-being through engaging conversations, companionship, and social activities aligned with their interests. Perform light housekeeping tasks such as cleaning living areas and laundry to promote a safe and comfortable environment. Requirements Proven experience in home health care, assisted living facilities, nursing homes, or group homes with a focus on long-term care or hospice care. Knowledge of patient observation techniques, medication administration, vital signs monitoring, and dementia or Alzheimer's care. Certifications in CPR and First Aid are required; additional training in HIPAA compliance is preferred. Ability to lift heavy objects safely using assistive devices like Hoyer lifts or transfer belts; physical stamina is essential. Experience working with individuals with developmental disabilities, autism spectrum disorder (ASD), or other special needs is highly valued. Strong communication skills to collaborate effectively with clients' families and healthcare teams. Flexibility to work various shifts including evenings, weekends, or overnight as needed.
