This flexible role offers the opportunity to provide vital assistance in various settings, including group homes, assisted living , and home and community care environments. As an on-call team member, you will be a vital part of creating safe, supportive, and engaging environments that promote independence and well-being for those we serve. Your energetic approach and attention to detail will help ensure high-quality care around the clock. Responsibilities Provide personalized assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly. Assist with medication administration following HIPAA guidelines to ensure privacy and safety. Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness in kitchen and dining areas. Support residents during community outings or social activities to foster engagement and socialization. Maintain accurate care plans, document patient observations, and adhere to all safety protocols including CPR and first aid procedures. Perform light cleaning duties to uphold a safe and hygienic environment for residents and staff. Preferred Skills Proven experience in group home settings, assisted living, or home health environments with a focus on patient care. Knowledge of working with individuals with intellectual disabilities. Certification in CPR and first aid; familiarity with HIPAA regulations for patient privacy. Ability to lift heavy objects safely using equipment like Hoyer lifts; strong physical stamina required for patient monitoring and caregiving tasks. Excellent observational skills for patient monitoring and recognizing signs of distress or health changes. Experience in medication administration, care planning, and working within long-term care or hospice settings preferred. Compassionate communication skills tailored to working with seniors or individuals with disabilities; patience and active listening are essential.
