This role offers an opportunity to make a meaningful difference in the lives of those with diverse needs, including seniors, individuals with disabilities, and those experiencing memory care challenges. The ideal candidate will be energetic, detail-oriented, and committed to promoting independence, safety, and well-being through personalized caregiving. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by taking vital signs and observing for any changes in condition or behavior Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary Administer medications accurately following care plans and HIPAA confidentiality standards Support residents with memory care needs, including Alzheimer's and dementia care strategies Operate equipment such as Hoyer lifts and ensure safe transfer techniques for heavy lifting or mobility assistance Maintain cleanliness of living spaces through light housekeeping tasks including cleaning and laundry Document patient observations, care activities, and updates in compliance with healthcare regulations Collaborate with healthcare professionals to implement personalized care plans and ensure optimal patient support Provide emotional support and companionship to foster a positive environment for clients Skills Proven experience in home health, assisted living, or long-term care environments Knowledge of patient monitoring techniques and vital signs assessment Familiarity with working with individuals with developmental disabilities, autism, or dementia-related conditions Certified in CPR and First Aid; additional certifications such as HIPAA compliance are preferred Strong communication skills to effectively interact with clients, families, and healthcare teams Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary Excellent organizational skills for managing medication schedules and care plans Compassionate demeanor with a passion for caregiving and supporting independence Experience working within hospice or palliative care settings is a plus Join our team to provide vital support that enhances quality of life!
