Job Description

At Right at Home, we believe exceptional care starts with an exceptional client experience. The Client Care Coordinator plays a key role in ensuring clients and their families feel supported, heard, and valued throughout their journey with us. This position serves as the primary point of contact for client communication, satisfaction, issue resolution, and service excellence. Hours are in-office or visiting clients Monday through Friday, 9a-5p. Competitive pay, PTO, benefits, 401K! The Client Care Coordinator works closely with clients, families, caregivers, office staff, and referral partners to build strong relationships, proactively address concerns, and ensure every client receives outstanding service. The ideal candidate is professional, highly organized, customer-focused, and thrives in a fast-paced environment where building relationships and solving problems are key to success.
Essential Duties & Responsibilities:
Serve as a primary point of contact for clients and families, ensuring a positive and responsive customer experience Welcome new clients and coordinate a smooth onboarding process Conduct client and caregiver introductions to foster successful relationships from the start Make regular satisfaction calls and in-person visits to ensure clients are receiving exceptional service Receive and professionally resolve client concerns, complaints, and service issues Investigate concerns, identify solutions, and follow through to ensure resolution and client satisfaction Collaborate with scheduling and caregiving teams to address service challenges and maintain continuity of care Support client retention efforts through proactive communication and relationship building Maintain accurate and timely documentation in WellSky/ClearCare Build strong relationships with clients, families, caregivers, referral partners, and community contacts Represent Right at Home with professionalism and empathy during difficult or high-stress situations

Requirements:
High School Diploma or GED Driver's License with a reliable insured vehicle 3 Years of Home Care, Home Health, or Hospice 3
Years of Customer Service Pay:
From $18.00 per hour
