Masterfully prepare and manage daily, weekly, and weekend schedules for our field staff. You ensure every client receives seamless, high-quality care by thinking three steps ahead of potential gaps. Recruitment, Screening & Onboarding:
Identify, screen, and vet Care Partners who genuinely embrace our organization's core values of compassion, care, and dedication. The Vital Communication Link:
Serve as the central nervous system of our daily operations. Accurately and promptly communicate scheduling changes between the office, caregivers, and clients with a warm, professional, and reassuring approach. Meticulous Record Maintenance:
Keep flawless digital logs, schedules, and profile updates for both clients and care partners. Your strict attention to detail ensures compliance, smooth operations, and a high standard of care. On-Call Rotation:
Participate in a weekly after-hours on-call rotation. This is where your problem-solving skills truly shine—responding calmly, empathetically, and efficiently to last-minute emergency calls to ensure our clients and care partners are always supported. Team Collaboration:
Partner closely with other management team and field staff to understand their unique needs and preferences, fostering a harmonious, supportive work environment. Willing to listen to feedback, suggestions, recommendations and make it your own without thinking that you are being micromanaged. Transparency & Accountability :
Provide honest communication when you need assistance or would like to move in a different direction in coordinating for your client. Ensure that all your work and follow up is communicated clearly and effectively with the care team. Take accountability for mistakes without making needless & time-wasting excuses. Skills & Qualifications Experience Required:
Minimum of 1 year of experience as a Home Care Scheduler or in a similar fast-paced coordination role (Home Care, Hospice, or Home Healthcare highly preferred). High Operational IQ:
Strong organizational skills with an elite eye for detail—especially when it comes to emails, texts, and official communications. Tech-Savvy Adaptability:
Comfortable and experienced navigating Microsoft Office and scheduling software (direct experience with systems like AxisCare or WellSky is a huge plus). Balanced Communication:
Exceptional interpersonal skills. Ability to communicate on the phone and via text in a warm, empathetic manner, while maintaining professional rigor in writing. The Logistic Chessmaster:

High School Diploma or GED required; College degree preferred. Preferred Credentials:
