This position is vital in helping our clients maintain their independence and dignity. Responsibilities Monitor clients' health status, including vital signs and overall well-being. Provide personal care assistance, including bathing, dressing, and grooming. Develop individualized care plans in collaboration with clients and their families. Assist with mobility and physical activities to promote independence. Educate clients and families on health management and wellness practices. Document all patient interactions and changes in health status accurately. Requirements Experience in home care or senior care settings preferred.
