This outside salesperson role is responsible for identifying prospective clients and patients in need of home health care services, facilitating the admissions process, and coordinating care and transitions from healthcare facilities into the home setting. This is a non-clinical, outside salesperson role. Find your sense of belonging at Wesley! You will provide continuing care through these responsibilities Essential functions of this position include the following: Develop professional relationships and maintain contact with key referral sources including physicians, hospital care coordinators, skilled nursing or rehabilitation centers, and other strategic referral sources to drive census (contracts) for home care, home healthcare, hospice, and other community-based services. Educate referral sources and discharge planners about home care, home healthcare, hospice, and other community resources and services available. Achieve sales-related activity goals and performance benchmarks. Through professional relationships and sales delivery, position Wesley ads the trusted provider of exceptional senior care. Conduct pre-discharge facility visits upon request to determine eligibility for home care, home health, or hospice. Demonstrate ability to screen patients for appropriateness to receive care for home care, home healthcare, hospice, and other community-based services. Communicate with hospital personnel, patient and/or patient family of case acceptance and document all demographic, clinical, payer and other information to support diagnosis and home healthcare orders. Provide input to care plan development and informs clinical staff (e.g. Intake Specialist, Director of Clinical Services and/or others as appropriate) of patient's acceptance, and ensure that patient's needs are appropriately matched. Assist clients, patients and family members to achieve smooth transitions from healthcare facilities to home care. Effectively communicate purpose and value of services in non-medical terms to foster patient acceptance and engagement with in-home skilled and non-skilled services. Learn and apply Company, Federal, State, and local rules, regulations, and policies; abide by all operational compliance with internal controls and reporting. Maintain market awareness and prepare competitive updates and reports as requested. Other duties and projects as assigned. Our requirements and qualifications for success Formal sales training preferred. Driving is an essential function and/or a legitimate business purpose of this position; therefore, a valid driver's license is required as a condition of employment. Prior knowledge of marketing strategy, tactics, and sales techniques. Proficient with Microsoft Office suite and other common workplace technology. Ability to read, write, speak and understand the English language. Ability to project professionalism in representing the company. Ability to assess clinical needs of referred patients to screen for eligibility and appropriate level of care. Outstanding verbal and written communication skills required. Ability to speak in group settings. Proficiency with electronic health record systems (e.g. Brightree, PCC) and other software programs. Proven track record of successful organizational skills, time management and follow-through. Ability to handle multiple projects simultaneously. Proven ability to achieve sales targets in home health-related field. Minimum of two years' experience in home health, hospice, geriatrics, or hospital case management experience. Minimum one years' experience in a direct or indirect sales role preferred. Physical, environmental, and mental requirements Ability to sit/stand or walk at-will throughout shift. Ability to hear, understand, and distinguish speech and/or other sounds one-to-one, in a group, and via telephone/computer. Keyboarding and near visual acuity. Ability to lift up to 25 lbs. Salary Range:
$72,627.36 to $104,677.66 per year
