This vital role involves assisting the two clients with daily activities, ensuring their safety, and promoting independence through personalized care plans by remaining in the home with the couple from 7am Saturday until 7am Monday. The ideal candidate will bring energy, empathy, and a proactive approach to enhancing the quality of life for those they serve. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe changes in health status to report promptly to office Assist with medications accurately following care plans to ensure patient safety Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary Use specialized equipment such as Hoyer lifts or transfer devices to safely move clients Maintain cleanliness of the client's environment through light housekeeping tasks Document patient observations, care provided, and any incidents in compliance with privacy standards Support clients during outings or community activities to promote social engagement and independence Implement specialized care techniques for dementia, Alzheimer's, memory care, or hospice needs as required Collaborate with healthcare teams to develop and follow personalized care plans that meet each client's unique needs Experience Prior experience working in home health, assisted living, nursing homes, or group homes is highly preferred Demonstrated ability to assist with ADLs and provide compassionate patient care in diverse settings Ability to lift heavy objects safely using equipment like Hoyer lifts when required for resident transfers Strong understanding of HIPAA regulations and maintaining confidentiality of patient information Background in social work or long-term care adds valuable insight into holistic client support Join us in making a meaningful difference that empowers individuals to live their best lives at home.
