Job Description

Advanced Care LLC is currently hiring for an Appointment Escort- Caregiver Associate. Our company is dedicated to helping adults and kids with developmental disabilities thrive and complete their personal goals. The main purpose of this position is to prevent our customers from being institutionalized. You will work one-on-one with adults with developmental disabilities to take them outside for social activity. Majority of our clients already have transportation provided. We work with clients in Shoreline, Lynnwood, Everett, Bothell, and Edmonds. Ideal staff will be able to work among these locations and a plus if able to take the few clients without transportation in own vehicle, reimbursement for mileage.
Requirements:
All requirements must be met. Preferred to have high school diploma or equivalent Previous companionship and personal care experience Passionate about child & elderly care with a genuine desire to help others Reliable and professional Preferred to have 1 year of experience Responsibilities Provide one-on-one services designed to help individuals with developmental disabilities increase their connection to and involvement in their community. Support clients in identifying and participating in preferred community activities, organizations, groups, or projects. Introduce clients to local community members, organizations, and available resources. Facilitate the development of positive relationships, meaningful roles, and inclusive community involvement. Encourage the use of both formal and informal community supports to foster long-term connections. Assist clients in building skills that promote independence and participation in community life. Collaborate with clients to set individualized goals that align with their Person-Centered Service Plan (PCSP). Maintain accurate, timely documentation of services provided and progress toward goals. Work collaboratively with families, caregivers, and other support team members. Ensure services are culturally appropriate and sensitive to individual needs.
