This role focuses on helping residents maintain their independence, dignity, and quality of life while ensuring their health, safety, and well-being. The ideal candidate is patient, respectful, and committed to providing person-centered care that empowers individuals to achieve their personal goals and participate fully in their community. Essential Duties and Responsibilities Provide assistance with activities of daily living (ADLs), including: Personal hygiene and grooming Bathing and dressing Toileting and incontinence care Feeding and meal assistance Assist residents with mobility, transfers, positioning, and use of adaptive equipment according to care plans and training. Administer medications as authorized and document administration accurately. Monitor and report changes in residents' physical, emotional, or behavioral conditions. Prepare meals and assist with meal planning according to dietary needs and preferences. Maintain a clean, safe, and organized home environment. Complete required documentation, progress notes, and incident reports accurately and timely. Provide companionship and emotional support while promoting independence and self-determination. Assist residents with recreational activities, appointments, and community outings. Follow all agency policies, safety procedures, and state regulations. Communicate effectively with residents, family members, healthcare providers, and team members.

Qualifications Required:

Must Be CBRF certified At least 18 years of age. Ability to pass background checks and any required screenings. Reliable transportation Ability to lift, transfer, and assist individuals with mobility limitations. Strong communication and interpersonal skills.

Preferred:

Previous experience as a caregiver, direct support professional, CNA, home health aide, or similar role. Experience supporting individuals with physical disabilities. Medication administration certification or willingness to obtain certification. CPR and First Aid certification (or willingness to obtain). Physical Requirements Ability to stand, walk, bend, stoop, and lift up to 40 pds Ability to safely assist with transfers and mobility support. Ability to respond quickly in emergency situations.
