This dynamic position offers the opportunity to provide comprehensive support in a home setting, ensuring safety, comfort, and independence for clients with diverse needs. As a dedicated caregiver, you will deliver attentive, respectful, and professional care while fostering trusting relationships that promote well-being and dignity. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by checking vital signs, observing symptoms, and reporting changes promptly Administer medications accurately following care plans and HIPAA regulations to ensure safety and compliance Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary Support clients with memory care, dementia, Alzheimer's, or autism by implementing specialized techniques and providing a calming environment Use equipment such as Hoyer lifts or resident lifts safely for heavy lifting or transfer needs Maintain cleanliness of the living environment through light housekeeping tasks such as cleaning, laundry, and organizing Follow detailed care plans created by healthcare professionals to deliver consistent and personalized support Document patient observations, progress notes, and incident reports meticulously in accordance with healthcare standards Engage in patient monitoring to ensure safety during activities and rest periods while respecting privacy and dignity Qualifications Proven experience in home & community care or assisted living environments; group home experience is a plus Knowledge of patient observation techniques, medication administration, and vital signs monitoring Certifications in CPR, First Aid, HIPAA compliance, and hospice or nursing home experience preferred Demonstrated ability to work with individuals with developmental disabilities, dementia, Alzheimer's disease or autism spectrum disorders Ability to perform heavy lifting safely using appropriate equipment such as Hoyer lifts or resident lifts Strong communication skills for working effectively with clients, families, and healthcare teams Compassionate attitude with a commitment to providing respectful and dignified care in a home setting Join us to make a difference every day!
