POSITION SUMMARY
Will be responsible for providing TABI case management services for Alaskans 18 years or older who experience a traumatic or acquired brain injury (TABI) to assist the individual in gaining access to needed medical, rehabilitative, social, educational, and other support services. The goal of these services is to support individuals living with the physical, cognitive and behavior problems resulting from TABI so that they may live in the community as independently and productively as possible. TABI case management will travel to and provide by tele-health care to villages regularly. Reports directly to the Senior & Disabilities Director.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Provides TABI case management services. Works with regional partners for making and getting referrals related to TABI case management. Attends annual State of Alaska TABI Conference Attends staff meetings and in-service trainings. Completes all required reporting forms correctly and in a timely manner. Provides and collects surveys regularly for evaluations on performance of TABI case management services. Assists the individual in gaining access to needed medical, rehabilitative, social, educational, and other support services. Assists people with traumatic and acquired brain injury and their families to achieve short-term and overall life goals, and will help to minimize the severity of disabilities through interventions and compensatory strategies. Maintains confidentiality of all client related information as per State and Federal Regulations, Maniilaq policies and procedures, and Staff Code of Conduct. Other duties as assigned.
Job Qualifications:
MINIMUM REQUIREMENTS
High school diploma or equivalent. Bachelor's degree in a related Social Services field or three (3) years of working with individuals with disabilities or related field. Ability to work in teams and ability to develop teams. Ability to work with minimal supervision. Ability and willingness to travel in small aircraft to villages within Maniilaq's service area. Ability to work with sensitivity in a cross-cultural environment. Ability to adjust to "bush" conditions in Arctic Alaska. Possess good communication skills and an aptitude for working with people.
