Job Summary We are seeking a dedicated and compassionate Case Manager/Service Coordinator to join our dynamic team. In this vital role, you will coordinate and deliver comprehensive support services to individuals and families facing complex challenges. Your energetic approach will help empower clients to achieve stability, independence, and improved well-being. You will be guiding them through their personalized care plans. This position offers an exciting opportunity to make a meaningful impact in the community while working in a fast-paced, rewarding environment. Duties Assess clients' needs through interviews and collaboration with multidisciplinary teams. Monitor client progress regularly, while maintaining detailed documentation of services provided. Collaborate with supervisors and team members to ensure quality service delivery, including supervising the clients caregivers. Qualifications Demonstrated ability in motivational interviewing, mentoring, and supervising clients services. Excellent communication skills with the ability to handle sensitive situations professionally and compassionately. Ability to work independently while collaborating effectively within a multidisciplinary team environment. Be willing and able to be trained.
