ACT Case Manager SeaView Community Services - Seward, Alaska Full-Time | Behavioral Health | Benefits Eligible About SeaView SeaView Community Services has been serving the Seward community since 1972, providing a full continuum of behavioral health, housing, and supportive services. Our Assertive Community Treatment (ACT) program is the largest outpatient program at SeaView and plays a vital role in delivering intensive, community-based care to individuals with serious and persistent mental illness. We are seeking a compassionate and dedicated ACT Case Manager to join our team. This position offers the opportunity to make a direct impact in the lives of clients by supporting recovery, independence, and improved quality of life. Responsibilities Provide intensive case management services as part of a multidisciplinary ACT team. Assist clients with accessing housing, employment, medical care, benefits, and community resources. Conduct home visits, community-based support, and outreach to ensure continuity of care. Support clients in achieving goals around daily living skills, wellness, and recovery. Document services in compliance with Medicaid and program standards. Collaborate with team leader, clinical associate, nurse, and psychiatrist to provide wraparound, person-centered care. Participate in weekly team meetings, crisis response, and 24/7 coverage rotation as assigned. Qualifications Bachelor's degree in Social Work, Psychology, Human Services, or related field (or equivalent experience) required. Previous experience working in behavioral health, case management, or social services strongly preferred. Ability to work independently and as part of a collaborative team. Strong communication, organization, and problem-solving skills. Valid driver's license and ability to travel within the community.
