Case Manager

The Bridge, Inc. - Alabama only - multiple locations As a Case Manager , YOU will provide case management services to adolescents and their families in our Transitions program. You will serve our Northwest AL (Fayette, Lamar, Marion Counties) programs at various locations in the community, schools, and homes under the supervision of the program manager. This is a full-time, non-exempt position, Monday-Friday . Great benefits, eligible mileage reimbursement, and a cell phone stipend! Requirements High school diploma or GED, Bachelor's degree preferred Valid Alabama driver's license and acceptable driving record with proof of personal vehicle insurance Have access to personal transportation to meet with clients Basic computer skills Job Responsibilities / Essential Functions Provide case management services including group and individual services for Basic Living Skills, Prevention, Parent Education, Family Support, Crisis Intervention, and other services Willing to work with families and clients during non-traditional hours in a variety of settings including office, school, client's home, or other approved community settings Assist in counselor/staff consultations on appropriate interventions with clients Work collaboratively with agencies and maintain a professional working relationship between the Center and other agencies Perform basic clerical activities Document activities and incidents Participate in staff meetings, training, and in-service meetings About us Founded in 1974, The Bridge provides substance use disorder treatment and behavioral rehabilitation in Alabama counties.
