Job Description

Schedule/Hours:
Monday through Friday, 8:00 am - 4:30 pm (40 hrs. per week) This position is responsible for: Interviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients in an established life management program. Develops program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff.
Key Responsibilities:
Conduct needs assessment to determine eligibility for program based on guidelines and requirements. Make referrals to other agencies if not appropriate for program. Prepares and maintains case records of assigned clients. Enters pertinent information into the Homeless Management Information System (HMIS). Establishes comprehensive goals for eligible clients May accompany and/or provide transportation for clients to meetings related to case plan.
Physical Requirements and Working Conditions:
Work is also performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
