Full-Time Position Description As a Case Manager , you'll provide a full range of direct and indirect mental health case management services to support the individuals we serve. You'll be a key part of the treatment team, helping to ensure care is coordinated, documented, and delivered with compassion and professionalism. This role is ideal for someone who thrives on collaboration, advocacy, and making a difference in the lives of others. What You'll Do Support the admission process, including preadmission activities and completion of admission documents. Provide individual case management and psychoeducational groups to meet the needs of individuals served. Coordinate care and services to ensure continuity and quality of treatment. Actively participate in treatment team meetings to support care planning. Document services in medical records accurately and in compliance with requirements. Assist with data collection, entry, and reporting to support program evaluation. Build and maintain positive professional relationships with internal and external stakeholders. Demonstrate cultural competency and age-appropriate approaches in all interactions.
