Aftercare Case Manager Bakersfield, CA 93301 $17

Full-time Fundamental Objective:
Under the direct supervision of the Milestone Aftercare Supervisor will provide supportive services to residents of the Milestone projects through a biopsychosocial and trauma-informed perspective.
Minimal Qualifications:
Possession of an B.A. or B.S. in Social Work/Sociology, Psychology, Human Services, or a related field is preferred, and reduces the required experience to one (1) years, or two (2) years of experience sufficient to obtain the knowledge and skill to successfully perform the essential duties of the job. Valid California Driver's License, car insurance and provide own transportation. 2 years free of domestic violence Bilingual English/Spanish preferred
Essential Job Duties and Responsibilities:
Knowledge of general office practice. Ability to work with clients from diverse socioeconomic backgrounds. Provide on-going comprehensive case management services for Milestone project residents through positive interactions and rapport building techniques. Offer service delivery based on evidence-based assessment tools, a biopsychosocial model and client input. Work as a team with other ODN staff to provide wellness groups, educational classes, and daily life skills. Practices excellent communication skills with clients, staff, and community partners. Understands the basics of trauma-informed care when working with clients. Understands the housing rental market and can provide housing search services for clients wanting to live independently. Acts as an advocate for client strengths and growth towards self-sufficiency and independent living. Attend all staff trainings and meetings for personal and professional as assigned by supervisor. Daily, weekly, and monthly data reporting and analysis of client services Regular auditing of case files in both digital and physical formats Attend and represent TODN at community events and collaborative committees as needed. Other duties as assigned.
