Position Summary:
Under the direction of the Street Outreach Supervisor, the Street Outreach Case Manager serves as a frontline representative of the organization by engaging unsheltered individuals in the community, building trusting relationships, conducting assessments, developing service plans, and connecting clients to housing and supportive services. This position primarily operates during the weekday/weeknight/weekday hours and requires extensive field-based work and regular interaction with vulnerable populations experiencing homelessness, mental illness, substance use disorders, trauma, and other barriers to stability. The Street Outreach Case Manager utilizes trauma-informed, harm reduction, and housing-focused approaches to support individuals in achieving self-sufficiency, housing stability, and improved quality of life.
Minimal Qualifications:
Possession of an Associate's Degree in Human Services, Social Work, Psychology, Sociology, Public Health, Criminal Justice, or a related field; or two (2) years of experience providing homeless services, outreach, case management, behavioral health services, or related social services; or an equivalent combination of education, training, and experience. Experience working with individuals experiencing homelessness, behavioral health challenges, substance use disorders, or other vulnerable populations preferred. Familiarity with homeless services systems, housing resources, public benefits, and community resources preferred. Knowledge of homelessness, housing barriers, and community-based service systems. Ability to work with individuals experiencing mental health challenges, substance use disorders, trauma, and crisis situations. Knowledge of trauma-informed care, motivational interviewing, harm reduction, and strengths-based service delivery principles. Ability to establish rapport and maintain professional boundaries with diverse populations. Strong communication, documentation, organizational, and time-management skills. Ability to work independently in field-based environments. Must be flexible to work weekdays, weeknights and weekends Possession of a valid California Driver's License and insurable driving record.
Essential Job Duties and Responsibilities:
Conduct street outreach activities in encampments, parks, shelters, hospitals, public spaces, and other locations where individuals experiencing homelessness may be found. Establish rapport and build trusting relationships with individuals experiencing homelessness through consistent and compassionate engagement. Conduct client intakes, assessments, housing assessments, vulnerability assessments, and service planning activities. Develop individualized service plans and housing-focused goals in collaboration with clients. Assist clients with obtaining identification documents, income verification, benefits enrollment, healthcare services, transportation, and other resources necessary to achieve housing stability. Provide housing navigation services including housing search assistance, landlord engagement, housing applications, lease-up support, and move-in coordination. Assist clients with referrals to emergency shelter, interim housing, permanent housing, behavioral health treatment, substance use treatment, medical services, domestic violence services, and other community resources. Coordinate services with community providers, healthcare systems, behavioral health agencies, housing providers, and public agencies. Maintain regular contact with assigned clients to monitor progress toward goals and ensure continuity of care. Participate in multidisciplinary team meetings, case conferencing, Coordinated Entry System activities, and other collaborative service planning efforts. Respond appropriately to crisis situations and coordinate emergency interventions when necessary. Accurately document all client interactions, assessments, service plans, referrals, case notes, and outcomes within required timelines. Maintain compliance with agency policies, funder requirements, confidentiality standards, and program procedures. Collect and report program data and client outcomes as required. Distribute outreach supplies, hygiene kits, food, water, transportation assistance, and other basic needs resources as available. Advocate on behalf of clients to remove barriers to housing, healthcare, employment, benefits, and other essential services. Participate in training, staff meetings, professional development activities, and community outreach events. Maintain professional relationships with community stakeholders and service providers. Ensure clients' rights, dignity, privacy, and confidentiality are protected at all times. Perform other duties as assigned.
Pay:
$18.00 - $20.00 per hour
