JOB TITLE
Transitional Housing Program SPA 7- Case Manager The primary responsibility of is to provide case management services in Transitional Housing services to 12 homeless male youth aged 18-24. The position will work closely with the Program Director and Supervisor to ensure the program meets contract and agency expectations. The goal of the program is to transition youth into permanent housing opportunities. From time to time, the Case Manager may be asked by the Program Management to conduct other activities or tasks necessary to the operations of the organization. Due to the nature of the program, flexibility in the schedule is required including availability to take phone calls during evenings/holidays/weekends from time to time to ensure contact and supervision of youth. DUTIES AND RESPONSIBILITIES
The Transitional Housing Program Case Manager will work as part of a team under the direct supervision of the Short-Term Housing Director and Supervisor. Coordinate intake of eligible clients. Assess clients' challenges and barriers as well as strengths. Elaborate with client his Individualized Service Plan that will be updated monthly. Provide case management services to achieve the outcomes set in the ISP. Provide clients with referrals and monitor progress as needed, in particular to mental health and substance abuse services, and toward opportunities for educational, career development, and personal enrichment. Evaluate transportation needs and support them in developing a sustainable plan. Provide clear direction, support and follow up to clients. Support clients in identifying permanent housing options. Develop appropriate procedures in case of crisis. Support procedures related to grievance management. Support program director in evaluating gaps in services and development of policies and procedure for the program Program Administration:
Maintain files in a secure and locked cabinet. Maintain files according to agency and contract standards. Collect client documentation. Enter client data into the HMIS system on an ongoing daily-to-weekly basis. Prepare and submit reports in a timely manner, including situations and events that may negatively affect program operations. Other:
Follow agency policies, procedures, and professional code of conduct at all times. Lead delegation of tasks and activities to other program staff according to program needs. This includes but it is not limited to collection of documentation, delivery of notifications, transportation needs, data entry, etc. Other duties and responsibilities may be developed by immediate supervisor according to program needs. Ability to reach with hands and arms. Ability to use computer keyboard up to 50% of the day. Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment. Ability to lift and/or move up to 20 pounds. Bachelor's degree or 4 years of experience in a relevant setting. Experience working with transitional aged youth, individuals with mental disabilities, low-income families. Demonstrated ability to work with a culturally diverse clientele & staff. Bilingual and fluent in English and Spanish a must Valid California driver's license and personal vehicle insurance carrier relating to clients, staff agency representatives, community members and others. This is a full-time position. Experience and degree will be taken into consideration. Salary range:
$ $47,840.00 - $58,240.00. Health, dental and life insurance provided, complementary vision insurance available. Paid time off, sick time and paid holidays are provided according to agency policies. Employee will be able to enroll in agency's 401k plan. Agency provides matching contributions according to agency policies. Must have your own car.
