Case Manager

Responsibilities Conduct comprehensive assessments to identify clients' needs and develop personalized care plans. Provide crisis intervention and management to ensure immediate safety and stability for clients experiencing urgent issues. Offer addiction counseling and behavioral health support, utilizing motivational interviewing techniques to encourage positive change. Facilitate CFT meetings for clients and supports . Collaborate with healthcare providers, social service agencies, and community organizations to facilitate access to resources. Maintain detailed documentation of client interactions, progress notes, and case management plans in compliance with organizational policies. Advocate for clients' needs within various systems Requirements Proven experience in crisis management, behavioral health, or social work fields. Strong background in crisis intervention, addiction counseling, and motivational interviewing techniques. Excellent communication skills with the ability to build trust and rapport with diverse populations. Ability to handle sensitive situations with professionalism and empathy. Relevant educational background in social work, psychology, counseling, or related fields is required. Must demonstrate flexibility, problem-solving abilities, and a commitment to ongoing professional development.
