Position Summary:
The Outreach/Case Manager is responsible for engaging, supporting, and providing intensive case management services to youth experiencing homelessness. This role focuses on community outreach, client engagement, service coordination, and ongoing support to help transitional-aged youth (TAY) achieve housing stability, personal development, and long-term self-sufficiency. The ideal candidate is passionate about working with at-risk youth, demonstrates a trauma-informed and strengths-based approach, and has strong knowledge of local community resources and support systems.
Key Responsibilities:
Conduct street-based and community outreach to identify and engage youth ages 16-24 experiencing homelessness. Build rapport with youth at shelters, drop-in centers, schools, encampments, and other high-need areas. Distribute basic necessities such as hygiene kits, food, and clothing to support initial engagement. Collaborate with community partners, probation officers, schools, and local agencies to identify eligible participants. Attend community events, meetings, and outreach activities to promote program services and build partnerships. Conduct intake assessments and evaluate client needs. Develop Individual Service Plans (ISPs) based on client goals and needs. Provide ongoing case management, including advocacy, crisis intervention, referrals, and consistent follow-ups. Support clients in accessing housing, employment, education, mental health, and substance use services. Monitor progress and update service plans as needed to ensure goal achievement. Maintain accurate and timely documentation, including case notes, assessments, and service records. Ensure compliance with confidentiality regulations (e.g., HIPAA) and agency policies. Complete required reports for internal use and funding compliance. Follow all applicable local, state, and federal regulations. Work closely with internal teams such as clinicians, housing specialists, and life coaches to provide coordinated care. Participate in team meetings, trainings, and supervision sessions. Coordinate with external service providers to ensure wraparound support for clients.
Qualifications:
Required:
Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field (or equivalent experience). 1-2 years of experience in outreach, case management, or a related social services role. Knowledge of trauma-informed care, harm reduction, and strengths-based practices. Strong communication, organizational, and interpersonal skills. Ability to work independently and as part of a team in a fast-paced environment. Proficiency in Microsoft Office and case management systems.
Preferred:
Bilingual (Spanish, ASL, or other languages). Experience working with youth, transitional-aged youth (TAY), or re-entry populations. Familiarity with local housing systems, public benefits, and community resources.
Work Environment & Physical Requirements:
Combination of field-based and office-based work. Regular community outreach in various locations. Must be able to travel locally (personal or agency vehicle may be required). Occasional evening or weekend availability depending on program and client needs.
