About the Role The Case Manager plays a key role in supporting individuals experiencing homelessness or housing instability. This position works closely with participants to help them navigate services, build stability, and achieve long-term housing goals. Using a client-centered and strengths-based approach, the Case Manager provides individualized support, helping participants access resources such as healthcare, employment services, benefits, and housing programs. The role focuses on empowering clients to overcome barriers and build pathways toward independence and stability. Key Responsibilities Conduct participant intakes and comprehensive needs assessments. Collaborate with participants to develop Individualized Service Plans (ISP) addressing housing goals, income, health, and other supportive needs. Provide referrals, linkages, and support to community resources and services. Complete program-specific assessments (e.g., vulnerability or housing prioritization tools). Monitor participant progress and conduct periodic service plan reviews. Maintain accurate and timely case documentation and participant records. Ensure confidentiality and compliance with applicable privacy and documentation standards. Build professional relationships with participants and community partners to support engagement and retention. Provide crisis intervention, safety planning, and risk assessment services when needed. Utilize trauma-informed care and motivational interviewing techniques in service delivery. Maintain up-to-date knowledge of community resources to provide comprehensive support. Preferred Qualifications Two (2) years of experience in case management, healthcare, homeless services, social services, or working with vulnerable populations. Minimum Qualifications Candidates with various levels of education and experience are encouraged to apply. All candidates must be able to: Verify employment eligibility Obtain CPR and First Aid certification (or be willing to obtain upon hire) Successfully complete: Tuberculosis test Background screening Drug screening Driving is an essential function of this role.

Have reliable transportation Possess a valid driver's license Provide proof of personal vehicle insurance and ownership if using a personal vehicle for work duties Be able to meet organizational insurance requirements #COR1

$26.00•$28.00 per hour
