$24-$28 per/hr About the Role A growing nonprofit organization is seeking a compassionate and motivated Case Manager to support individuals experiencing homelessness. The Case Manager will use a "whatever it takes" approach to help clients access services, increase stability, and transition into permanent housing. This role involves field work, client support, documentation, and collaboration with community partners. Key Responsibilities Conduct client intakes and complete individualized needs assessments. Develop and maintain Individualized Service Plans (ISPs) focused on housing, income, health, and personal goals. Provide referrals and link clients to community resources including healthcare, benefits, mental health, and employment services. Complete program assessments and ongoing goal reviews. Maintain accurate, timely case notes and participant files in accordance with confidentiality standards. Build supportive, professional relationships with clients to promote engagement and retention. Provide crisis intervention, safety planning, and problem-solving support. Utilize trauma-informed care, motivational interviewing, and harm reduction practices. Attend required trainings, team meetings, and community coordination events. Support a safe, inclusive, and equitable service environment. Preferred Qualifications 2+ years of experience in case management, homeless services, healthcare, social services, or related fields. Experience working with vulnerable or high-needs populations. General Requirements Ability to obtain CPR/First Aid certification (employer-provided). Ability to work flexible hours, including some evenings or weekends. Must pass background checks, drug screening, and TB test. Must be able to work onsite within California. Driving Requirements Driving is an essential function of this role.
Candidates must:
Hold a valid CA driver's license. Have access to a personal vehicle. Maintain personal auto insurance meeting CA requirements. Be eligible for employer insurance coverage. $24.00 - $28.00 per hour
