Our mission is to help homeless youth and at-risk families become productive and integrated members of the community. We provide emergency and permanent housing options and other community-based services that create opportunities for them to lead productive lives. We operate in Los Angeles and community East and Southeast of Los Angeles County.
JOB TITLE
Bridge Housing Program- SPA 7- Case Manager II The primary responsibility of the case manager is to will provide case management services to 20 homeless female youth age 18-24. The case manager will be part of a team that will include staff assigned to monitoring the facility 24-7. The position will work closely with the program Director to ensure the program meets contract and agency expectation. The goal of the program is to transition youth into permanent housing opportunities. From time to time, the case manager may be asked by program manager or executive staff to conduct activities or tasks necessary to the operations of the organization. Due to the nature of the program, flexibility in the schedule is required including availability to take phone calls during evenings/holidays/weekends from time to time to ensure contact and supervision of youth.
DUTIES AND RESPONSIBILITIES
The Bridge Housing Program Case Manager II will work as part of a team under the direct supervision of the Short-Term Housing Director. The following reflects management's definition of essential functions for this job but does not restrict the tasks that may be assigned. Program:
Coordinate intake of eligible clients. Assess clients' challenges and barriers as well as strengths. Elaborate with client his Individualized Service Plan that will be updated monthly. Provide case management services to achieve the outcomes set in the ISP. Provide clients with referrals and monitor progress as needed, to mental health and substance abuse services, and toward opportunities for educational, career development, and personal enrichment. Evaluate transportation needs and support them in developing a sustainable plan. Provide clear directions, support and follow-up to clients. Support clients in identifying permanent housing options. Develop appropriate procedures in case of crisis. Support procedures related to grievance management. Coordination Support program director in evaluating and developing opportunities to improve operations across the agency short term housing program in SPA 4. Support program director in evaluating gaps in services and operations.
Administrative:
Maintain files in a secure and locked cabinet. Maintain files according to agency and contract standards. Collect client documentation. Enter client data into the HMIS system on an ongoing daily-to-weekly basis. Prepare and submit reports in a timely manner.
Other:
Follow the agency professional code of conduct at all times. Support scheduling of all staff in the program Support development of policies and procedures for the program. Coordinate other program staff in service delivery and facility monitoring. Manage scheduling of subordinate staff, under the supervision of program manager/Director. Delegate activities and follow up with other program staff to maximize clients' progress. Implement and or develop as needed, policies and procedures for all aspects of the program. Provide support and coaching to other case managers in program team. Other duties and responsibilities may be developed by immediate supervisor according to program needs.

Ability to effectively present information and respond to questions from clients, office personnel, other employees and the general public. Ability to read, analyze and interpret manuals and office documents.
Mental/Interpersonal Skills and Abilities:
Ability to define problems, gather, transcribe and post data, analyze, synthesize and classify information, and draw valid conclusions. Ability to work independently and with others, including other employees, clients and members of the public—in face-to-face and telephonic contexts. Ability to multi-task, establish priorities and manage time, meet deadlines, and perform duties under time constraints. Ability to be flexible and adapt to changing work demands. Maintain high level of concentration and attention to detail for extended periods of time. Ability to respond effectively to sensitive inquiries or complaints. Maintain a high level of ethical and professional standards in accordance with agency and community policy.
Physical Skills and Abilities:
Ability to talk or hear in order to give and receive information and instructions. Ability to stand, walk, sit, use hands to finger, handle or feel objects, tools or controls. Ability to reach with hands and arms. Ability to use computer keyboard up to 50% of the day. Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment. Ability to lift and/or move up to 20 pounds. Equipment and Computer Software Skills and Abilities:
• Computer literate (basic working knowledge of Microsoft WORD, Excel, Outlook, and PowerPoint) • Able to use telephone, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required
EDUCATION, EXPERIENCE, CERTIFICATES & TRAINING
Bachelor's degree or 4 years of experience in a relevant setting. Experience working with transitional aged youth, individuals with mental disabilities, low-income families. Demonstrated ability to work with a culturally diverse clientele & staff. Bilingual and fluent in English and Spanish a must Valid California driver's license and personal vehicle insurance carrier relating to clients, staff agency representatives, community members and others. This is a full-time position. Experience and degrees will be taken into consideration.
Salary range:
$52,000.00- $60,320.00. Health, dental and life insurance provided, complementary vision insurance available. Paid time off, sick time and paid holidays are provided according to agency policies. The employee will be able to enroll in agency's 401k plan according to agency policies. Must be willing to use your car for work-related travel. Mileage reimbursement provided.
