SUMMARY OF DUTIES
Under the immediate direction of the Project Director and/or Deputy Director, the Supervising Case Manager will coordinate and supervise Case Management services, in order to provide and assess appropriate preventions and interventions for GRYD youth and families. The Supervising Case Manager will develop detailed Case Management Service Plans, based on intake and assessment tools, provided by the Mayors GRYD Office. The Supervising Case Manager will supervise all Case Management related services, therefore assuring the highest quality of care to the youth, families and community we serve. In addition, the Supervising Case Manager will facilitate linkages between families and community resources, which will assist family members to develop the necessary skills for self-empowerment and self-sufficiency.
EXAMPLES OF DUTIES AND RESPOSNABILITIES
The Supervising Case Manger will develop and assist in the implementation of detailed service plans, for program participants and their families, who are identified as Secondary (Eligible/Model Clients). Service Plans will be based on information obtained by the Case Manager during the initial family meeting and the YSET- intake feedback report, as it relates to the identified risk factors. Maintain and enforce strict rules of confidentiality between GRYD Staff, clients and their families. Provide appropriate referrals to meet client and family needs. Provide and implement an ongoing 12-week Anger Management/Conflict Resolution program, based on evidence based curriculum. Conduct weekly Case Conferences and ongoing Case Management Trainings.
EXAMPLES OF DUTIES AND RESPOSNABILITIES
: Oversee and assure Case Managers comply with the Scope of Work, the Policies and Procedures Handbook and are up to date with Data inputting. Provide one on one counseling/therapy sessions, with GRYD Clients, on a case basis, depending on the needs of the family. Will lead a Strategy Team group to assure that the following areas are identified and targeted: problem areas, strategies that will reduce risk factors and the development of genograms which will identify strengths within the family and therefore create an Individualized Service Plan for all Secondary Clients. Participate in weekly GRYD Administrative meetings, Mayors GRYD office Trainings and other program related events. Other responsibilities as assigned by the Project Director and/or Deputy Director
QUALIFICATIONS AND EXPEREINCE
At least four years of case management supervision experience working with at-risk youth and families. Must provide leadership and structure that will enhance the Case Management department. A valid Class C Driver's License with personal liability insurance for a dependable vehicle is required.
Education :
A graduate degree from an accredited college or university, preferably in one of the following areas: Social Work, Mental Health, Psychology or any behavioral Science.
