We specialize in delivering compassionate, person-centered care through a variety of programs, including: Adult Family Home Agency (AFHA) Independent Living Services (ILS) Supported Living Services (SLS) Day Program Services Coordinated Family Support Services Our mission is to empower adults with developmental disabilities to live meaningful, independent, and fulfilling lives while receiving the support they need. We're looking for dedicated team members who share our commitment to person‑centered care and community support. Primary Purpose As a case manager, you will provide guidance, resources, and support to persons served in their homes, day programs, and community settings. Your role ensures that each individual receives person‑centered services in a safe, respectful, and empowering environment.

Maintain strict confidentiality of consumer, employee, and agency data.

Collaborate effectively while exercising independent judgment with minimal supervision.

Foster positive relationships with consumers, families, guests, and community partners.
Professional Standards:
Meet requirements for training, attendance, and dress code.
Technology Use:
Utilize computer software for documentation and communication. Duties & Responsibilities (not limited to) Coordinate and facilitate Individual Service Plan (ISP) meetings (annual, semi‑annual, quarterly, 30‑day, and special staffings). Develop and revise ISP goals using person‑centered planning tools. Assist in communication with day programs, employers, healthcare providers, families, Regional Center, Social Security, Medi‑Cal, and other agencies. Provide on‑call support to clients and families when needed. Maintain professional communication with Regional Center service coordinators and agency supervisors. Complete ISP reports and track client progress. Attend agency meetings and trainings as required. Keep client records updated and maintain accurate notes for each visit or correspondence.
Qualifications & Requirements:
Education/Experience:
Bachelor's Degree (preferred) OR minimum 2 years of experience working with individuals with developmental disabilities, including those with significant behavioral challenges.
Technical Skills:
Intermediate computer proficiency; ability to navigate case management software, email programs (PC & mobile), and multitask effectively.
Typing Ability:
Minimum of 40 words per minute.
Licensing & Transportation:
Valid California Driver's License, safe and reliable vehicle, and current auto insurance (mileage reimbursed for travel within Orange County). This role requires travel within south/north Orange County to conduct client home visits.
Age Requirement:
Must be at least 21 years old.
Background Clearance:
Successful completion of Live Scan fingerprinting and criminal records clearance.
Certifications:
CPR/First Aid certified (or willingness to obtain).
Preferred Experience:
Familiarity with Regional Center processes.
Language Skills:
Bilingual (English/Spanish) strongly preferred . ASL (American Sign Language) : +++ but not required
