Job Description

Overview
JOB SUMMARY
The Case Manager P.T. facilitates patient admission from outside referrals, provides on-site evaluation and provides ongoing communication to the external case manager on the progress of the patient. He/She assures that each patient is appropriate for admissions and writes treatment plan to external case managers. The Case Manager P.T. acts as liason between therapist, physicians, family/patient, and third party payors. The Case Manager P.T. participates in marketing of the program, obtaining input from the persons service and the ongoing program evaluation, quality improvement and utilization review systems.
EDUCATION, EXPERIENCE, TRAINING
Bachelor's degree preferred and appropriate licensure in Social Work, Physical Therapy, Occupational Therapy, Nursing or Speech Pathology, with hospital and/or rehabilitation experience ( 2 years) including utilization review knowledge. The Admissions/Case Coordinator reports to the Director of Rehabilitation Services. Awareness of Medicare, Medi-Cal and managed care guidelines for rehabilitation services. Must be able to see and hear staff, patients, physicians, etc.; view computer screens and documentation in medical record. Must be able to hear conversation/discussion with physicians and staff. Must be able to communicate orally to staff, patients, doctors, family members. Visually, employee must be able to read reports.
