Job Description

Case Manager Supervisor Job Summary Direct and supervise the case management program and personnel including but not limited to, all activities of case management include such practices as enrolling clients into the program, assessing needs, developing care plans, authorizing and coordinating services among providers, and providing follow-up and reassessment, as required. . Approximately 20-25% of your time will be spent conducting client assessments as needed in the program. This position will be responsible for the supervision of students and interns as applicable. This position will manage and develop program policies and procedures to improve the compliance and efficiency of our contracted programs. You assist the Program Administrator and Office Manager in insuring all client data is documented and recorded in our Agency systems and appropriate County database.
Essential Job Duties :
Specific duties and responsibilities may include, but are not limited to the following: Lead & manage the case management program with the Program Administrator, for the senior nutrition program that both meets the basic nutritional assessment requirements of: FSA, the OAA (Riverside County) and DAAS-PG (San Bernardino County) contracts and also provides resources and referrals to assist senior clients in meeting their needs. Supervise and coordinate the case work, nutrition quarterly assessments for senior nutrition program both counties (Riverside and San Bernardino). Screening, training, and orientation of all new managers including nutritional assessment paperwork, case management and referrals for seniors and home visiting safety procedures as per FSA procedures and policies. Assignment of cases and supervision and consultation of assigned cases, oversight and review of required program documentation is completed within required timeframes and guidelines. Supervises case managers and other FSA staff as assigned. Supervision of students and interns as applicable. Conducts a minimum of25% of work time in direct time with clients providing case management services. Knowledgeable of funding source (County) and Federal Title Ill Elderly Nutrition Program requirements Works with support staff to ensure that nutritional assessment data is completed timely. Coordination of ongoing case manager training and development in collaboration with program managers and administrator. Represents I leads agency at events or community meetings as assigned. Assists Nutrition Program Administrator for any program level efforts as assigned. Assists program administrator, ADM-Contracts team, and others in program development and grant writing as assigned for additional financial support for case management services for seniors. Maintains a consistent tracking system for client wait list for the Home delivered C-2 program Assist in Continuous Quality Improvement (CQI) process as needed to improve department and Agency processes, policies and procedures.
