Job Description

•Mental Health Primary certification equivalent is accepted! LUCID Oceanside is a growing substance abuse and mental health treatment center dedicated to providing compassionate, individualized care. We believe in empowering our clients on their journey to recovery, and we are looking for a dedicated Lead Case Manager to join our team. This role offers the opportunity for significant growth and advancement within our organization.
Overview:
The Lead Case Manager will be the primary point of contact for all patient needs, ensuring that each client receives exceptional care and support throughout their treatment journey. This role is crucial in maintaining a positive and healing environment, where customer service is a top priority. Additionally, the Lead Case Manager will facilitate group sessions, providing clients with the tools and support they need for recovery. As a key member of our team, you will play a vital role in shaping the future of our facility.
Main Responsibilities:
Serve as the primary contact for patients, addressing their needs, concerns, and questions with empathy and professionalism. Develop and manage individualized care plans, coordinating services and resources to support client recovery. Facilitate group therapy sessions, using evidence-based practices to engage and support clients in their treatment. Collaborate with clinical and administrative staff to ensure seamless care delivery. Monitor and document patient progress, making adjustments to care plans as needed. Advocate for patients, helping them access resources and navigate the recovery process. Participate in team meetings and contribute to the ongoing development of facility programs and services. Assist in training and mentoring new case management staff. Maintain compliance with all regulatory requirements and ethical standards.
Qualifications:
Experience in case management, preferably in a substance abuse or mental health setting. Strong customer service skills, with a passion for helping others. Experience in group facilitation, with knowledge of evidence-based practices. Excellent communication and interpersonal skills. Strong organizational and time-management abilities. Ability to work independently and as part of a team. Bachelor's degree in Social Work, Counseling, Psychology, or a related field is preferred but not required. Bilingual (English/Spanish) is a plus.
