  • 58K annually Direct Hire About the Role We are seeking a compassionate and driven Case Manager to support high-need populations through CalAIM initiatives, including Enhanced Care Management (ECM) and Community Supports.
This role focuses on improving client stability by connecting individuals to housing, healthcare, and essential community resources. This is a rewarding opportunity to make a meaningful impact while working with a mission-driven organization in Orange County. Key Responsibilities Conduct client assessments and develop individualized care plans Manage a caseload of approximately 25-60 clients Coordinate housing, medical, and behavioral health services Perform field outreach, including home visits and community-based support Maintain accurate documentation and ensure compliance with program requirements Collaborate with internal teams and external providers Provide crisis intervention and ongoing advocacy for clients Qualifications Bachelor's degree in Social Work, Psychology, Public Health, or related field (preferred) 2-3+ years of case management experience Experience working with underserved or high-need populations Familiarity with CalAIM or ECM programs is a plus Valid driver's license and willingness to travel locally Why Apply?
