Job Duties:
The case manager will be providing referred youth will the appropriate linkages and supportive services to meet needs that will assist with barriers. Conduct comprehensive screenings and assessments with youth and families to develop appropriate case plan. Act as advocate for youth before, judicial, community, social service and administrative bodies as needed. Will work in collaboration with other team members to ensure best possible outcome for youth. Weekly check
  • ins with youth and family to monitor youth progress and meet goal outcomes Maintains youths confidentiality per federal standards and all applicable code of ethics.
Meet with Program manager to evaluate referrals and outcomes. Candidate must have basic understanding of Restorative Justice concepts & practices and understanding of juvenile justice diversion programs Applicant must understand youth development principles and practices and ability to perform initial assessments of youth and family needs and develop services plans Ability to project self as a positive role model in dress, behavior, including maintaining professional limits and boundaries
Qualifications:
  • Must possess at least 3 years of experience working with at risk youth.
  • Must successfully pass background (Live scan, education, employment)
  • Proficient in Microsoft Office and Google Suite.
  • Must possess excellent verbal, written, and communication skills.
  • Be able to be well organized, work with minimal supervision.
  • Possess sensitivity to the service population's cultural and socio economic characteristic.
  • Ability to organize, multitask, prioritize duties and responsibility efficiently.
  • Ability to manage cases that include crisis intervention, traumatized families and youths mental health.
Education:
  • Minimum Bachelors Degree from Accredited University
  • Masters Degree (Highly Desired)
Experience:
  • Minimum of 3 years working as a Case Manger with at risk youth.
