starting from $20 Overview We are seeking a compassionate, organized, and bilingual Social Work Assistant (SWA) to join our Adult Day Health Care (ADHC/CBAS) team in Pomona. Under the supervision of the Social Worker and as part of the interdisciplinary team, the SWA assists with participant support services, care coordination, community resource referrals, documentation, and participant/family communication. The ideal candidate is committed to providing culturally sensitive care and support to seniors and adults with disabilities in a community-based healthcare setting. Responsibilities Assist the Social Worker with participant assessments, reassessments, individualized plan of care implementation, and follow-up services Support participants and family members by providing information regarding community resources, public benefits, and supportive services Assist with care coordination, referrals, transportation arrangements, and appointment scheduling as directed by the interdisciplinary team Maintain accurate, timely, and confidential participant documentation and case notes in compliance with HIPAA and California

regulations Conduct participant check-ins and provide psychosocial support under the direction of the Social Worker Assist with participant intake, enrollment, and discharge processes Report participant concerns, changes in condition, safety concerns, or psychosocial issues to the Social Worker and interdisciplinary team promptly Collaborate with nursing staff, rehabilitation therapists, activity staff, dietitians, caregivers, and other interdisciplinary team members to support participant well-being Participate in interdisciplinary team meetings, care conferences, and family meetings as assigned Maintain participant dignity, confidentiality, participant rights, and culturally competent communication at all times Perform other related duties as assigned within the scope of the position Requirements Bilingual in English and Spanish required Bachelor's degree required in Social Work, Human Services, Psychology, Sociology, Gerontology, or related social science field Minimum one 1 year of experience in healthcare, adult day health care (ADHC/CBAS), senior services, social services, case management, or a related setting preferred Knowledge of community resources, public assistance programs, and supportive services preferred Strong communication, interpersonal, organizational, and documentation skills Ability to work effectively as part of an interdisciplinary team in a fast-paced healthcare environment Compassionate, professional, patient, and culturally sensitive approach when working with elderly and disabled populations Basic computer proficiency, including electronic health records (EHR) and Microsoft Office applications Must meet all California Department of Public Health (CDPH) requirements, including criminal background clearance, health screening, and TB testing
