JOB SUMMARY
This person will act as the primary point of contact to ensure whole-person reentry services are provided as outlined by the California Advancing and Innovating Medi-Cal (CalAIM) Justice Involved (JI) policy. The case manager will work, as appropriate, with other providers, including Correctional Facilities (CF) providers, post release Enhanced Case Manager (ECM) Providers, county behavioral health agency providers, and community-based resources. The Pre-release Case Manager will support accessing and linking patients to resources in the community. The Case Manager will be responsible for screening individuals for pre-release services and developing a person-centered reentry care plan for all individuals receiving pre-release care management. The Case Manager will work closely with the patient's medical team, community non-profits, hospitals, and health clinics to coordinate access to primary health care. The Case Manager will also be responsible for following the Shasta Community Health Center (SCHC) protocol and practices around billing, documenting case notes, and entering data regarding patient progress in required Case Management system. This position is responsible for working with individuals with complex health and psychosocial needs. This position is responsible for conducting a warm handoff with the individual prior to release. The warm handoff is the first step in establishing a trusted relationship between the individual and the new care manager to ensure seamless service delivery and coordination.JOB DUTIES AND RESPONSIBILITIES
- Screen individual for Medi-Cal enrollment during/near intake
- Verify Medi-Cal enrollment through Partnership Provider Portal Services
- Support Needs Assessment
- obtaining consent to access and share any needed medical records with community-based providers/health plans, and coordination and support of delivery of services by CF clinical staff.
- Support Coordination of Care
- coordinate all needed care as part of the reentry stabilization, treatment, and planning for release.
- Support Reentry Care Plan Finalization, Warm Handoffs (for care management and behavioral health links), and Reentry Continuity of Care Plan
- Develop, implement, and monitor personalized re-entry care plans in collaboration with patients, healthcare providers, and community resources.
- Act as a patient advocate, ensuring the patient's needs and preferences are central to care decisions.
- Provide accurate daily and monthly documentation and case notes as per SCHC protocol and practices and funding source guidelines.
- Provide data sharing information that aligns with Department of Health Care Services (DHCS) policy changes
- Work as an integrated team with Medical and Correctional Facility; along with working with other counties that they receive youth from to help coordinate services.
- Work with individuals who are part of Justice Involved population

- Knowledge of mental illness issues and sensitivity towards issues of diversity
- Knowledge and ability working with youth with Justice Involved, Mental Health and substance use issues
- Ability to work independently and as part of a team
- Ability to manage a complex case load
- Excellent verbal and written communication skills
- Computer literacy
- Strong organizational skills
- Ability to effectively present information, respond to questions, and generally communicate clearly and respectfully with participants, coworkers, and other agencies.
- Comfortably discussing sensitive health issues with clients
- Strong interpersonal skills. Ability to work with people with a variety of backgrounds and educational levels both in person and on the phone
- Good judgment, problem solving and decision-making skills
- Ability to uphold patients' confidentiality with the utmost professionalism
- Ability to travel to other offices and community locations
- Support patients in a non-judgmental way.
EDUCATION & EXPERIENCE
- High school diploma or equivalent
- Must possess a valid CA DL and meet requirements of SCHC Auto Policy.
- CPR/BLS Certification upon hire and remain current.
PREFERRED QUALIFICATIONS
Two-Year associate degree or equivalent experience with Justice Involved Youths- Experience working with the Youths in Juvenile Rehabilitation Facility (JRF)
- Previous experience working in an outpatient clinic, doctor's office, or programs for mentally ill, SUD clients
- Knowledge of Homeless Management Information System (HMIS)
- Knowledge of harm reduction practices or theories
- Knowledge of Housing First principles
