As we grow to meet the evolving needs of those we serve, the dedicated Bi-Lingual Care Manager is instrumental in planning, organizing and managing wraparound services and providing resources with the Family Resource Center staff that support our mission-driven work. About the Role The Bi-Lingual Care Manager provides a key role in supporting clients and families through coordination of care, resource navigation, advocacy and culturally responsive support services. This position provides case management, parenting education, home visitation services and is instrumental in Cal-AIM-aligned initiatives such as Enhanced Care Management (ECM) and Community Supports (CS). The Care Manager ensures that clients receive high-quality services that promote stability, well-being, and improved quality of life. This role also participates in key outreach and community engagement programs that provide support to individuals and families through tangible resources, educational events, and seasonal donation drives. The Bi-Lingual Care Manager ensures that services are trauma-informed, culturally responsive, and aligned with the mission of Lighthouse Counseling and Family Resource Center to help individuals and families heal through counseling, education, and access to resources. Bi-lingual fluency (verbal and written) in Spanish and English is required.

Family Resource Manager Key Responsibilities:
Client Support & Case Management Conduct outreach and intake for new clients. Perform comprehensive assessments to identify client strengths, needs, and goals. Develop individualized care plans in collaboration with clients and interdisciplinary team members. Provide ongoing case management, follow-up, and care coordination. Facilitate connection to community-based services including housing, healthcare, counseling, food, employment, and educational opportunities. Provide culturally and linguistically appropriate services that respect background, traditions, and language preferences of clients. Conduct home visits as part of client service plans or as required by specific programs (e.g., home visitation programs, Path 1 and Path 2 referrals). Receive and respond to Path 1 and Path 2 referrals; engage families to provide assessment, home visitation, and supportive services. Support clients in addressing safety, stability, and overall family well-being. Ensure services and resources support whole-person care, address social drivers of health, and are compliant with Medi-Cal managed care plan requirements. Maintain support and care plans that are aligned with Family Resource Center policies and procedures. Enhanced Care Management (ECM) and Community Supports (CS) Implement CalAIM programs and documentation requirements. Participate in training on ECM and CS, home visitation best practices and Path referral protocols. Maintain accurate client records in alignment with program guidelines and funding requirements. Support program development and quality improvement efforts. Outreach and Community Engagement Participate in key community outreach events, health fairs, school-based events, and with other resource agencies within Placer County. Represent Lighthouse in CalAIM advisory groups, community coalitions, and local collaboration meetings (as appropriate). Assist with coordination and participate in core community building initiatives including: Car seat safety checks and installations Baby supply and diaper donation drives Backpacks and school supplies for children Holiday donation programs for families Work closely with FRC Manager and team to support outreach and seasonal giving campaigns. Provide support to Lighthouse staff who facilitate groups. Client Services and Case Management Oversight Provide quality connections to clients with intake, assessments, care planning, and follow-ups to ensure consistent, high-quality service delivery and outcomes. Work with Family Resource Center Manager and team regarding complex individual and family needs, ensuring equitable access to resources and long-term support to clients. Maintain accurate and timely documentation in electronic records systems. Track and report program outcomes and client progress. Participate in regular team meetings, supervision, and professional development activities. Uphold confidentiality and adhere to Lighthouse policies and procedures.
Qualifications Required:
Bi-lingual fluency in Spanish and English (written and verbal) Bachelor's degree in social work, public health, human services, or related field; equivalent experience will be considered. Minimum 1-2 years in case management, care coordination, home visitation or related experience in family support or social services. Experience working with diverse, low-income, and/or underserved populations. Strong understanding of local community resources and public benefit systems. Ability to work independently and collaboratively in a team-based environment. Knowledge of Medi-Cal, CalAIM, or whole-person care models. Excellent communication, organizational, and time-management skills. Proficiency with Microsoft Office and electronic case management systems. Strong problem-solving skills and ability to work independently, while living the values of the organization. Valid CA driver's license, insurance, and reliable transportation required for home visits, site-based work, and community events. Committed to deepening the culture of connectedness with staff, leaders and partners in dynamic internal and external environments. Experience with CalAIM initiatives, home visitation and serving trauma-informed populations. Prior experience working in a nonprofit, healthcare or community-based organization. Experience managing outreach events, community partnerships, or donation drives. Familiarity with ECM and CS provider networks, managed care plans, and local health initiatives.
