Behavioral Health Case Coordinator:

temp-to-hire opportunity for a highly driven, caring Behavioral Health Case Coordinator.
Position:
Behavioral Health Case Coordinator Pay Rate:
$25.00-$26.00 per hour
Schedule:
Full-Time, Monday-Friday, 8:30 AM - 5:00
PM Location:
San Diego, CA 92123 Bilingual in
Spanish Highly Preferred Role Overview:
As Behavioral Health Case Coordinator, you will: Develop personalized care plans and coordinate member services. Collaborate closely with members and families to ensure they receive necessary medical treatment and support. Consult with members to determine their needs, develop individualized action plans, and work with care teams to manage the member experience effectively. Provide emotional support, resolve administrative issues, and ensure timely access to care.
Key Responsibilities:
Conduct assessments and screenings to determine clients' needs and develop appropriate treatment goals. Monitor clients' progress and adjust care plans as needed for optimal outcomes. Maintain accurate and timely documentation of client interactions and treatment plans. Advocate for members, consult with healthcare providers, arrange appointments and treatment plans, evaluate member progress, and assist with healthcare barriers. Maintain empathy and professionalism while contacting members and families. Meet members in person once per month (or more frequently as needed).
Requirements:
Valid California driver's license and vehicle insurance. High school diploma or GED. Minimum of 1 year experience in case management , member care, customer service, call center, or related fields. Excellent communication, interpersonal, customer service, and organizational skills. Proficient in computer skills for documentation, email, and chat support. Ability to work independently and collaboratively in a team. Strong multitasking, organizational, and attention-to-detail skills. Ability to utilize resources effectively for problem-solving while maintaining composure under pressure. Proficiency in speaking, reading, and typing in both Spanish and English.
Additional Details:
You will be expected to meet with your members once a week or more, with efforts to group caseloads within the San Diego area or as close as possible.
