Job Description

Case Manager Bilingual CRF Behavioral Healthcare Overview Description Hours :
40 hours per week.

Clinic hours are Monday, 10:00 am to 6:00 pm, Tuesday, 11:00 am to 7:00 pm, Wednesday, 10:00 am to 6:00 pm; Thursday, 12:00 pm to 8:00 pm; and Friday, 9:00 am to 5:00 pm.
Pay Rate:
$22.00 to $24.00 per hour
Education :
Bachelor's degree with experience in a mental health related field, such as Psychology, Social Work or Counseling is required; Child and Adolescent experience preferred Bilingual Spanish is required for this position. As a Case Manager, you will: Promote healthy development and positive outcomes for families through utilizing case management and rehabilitative strategies with a team of dedicated professionals Assist clients and families to build upon natural support systems and links individuals to other service agencies. Provide additional supportive services to assist with continuity of treatment, linkage to resources, transition from program to program, and transition from program to community resources Connect every client/family to a medical home as well as clinic, school, home, and community-based services Serve as an advocate for the client and family Create and maintain all required documents in the electronic and paper record systems Additional Qualifications and Skills Experience working with children and adolescents with severe emotional disturbance in a public mental health setting Knowledge of County Health and Human Services requirements including documentation preferred Ability to make assessments, design treatment plans and document accordingly Ability to organize and prioritize multiple concurrent assignments and meet productivity goals Proficient computer skills, including MS Office A valid California drivers license in good standing, proof of insurance, and personal vehicle are required to fulfill the essential functions of this position. Ability to pass criminal/background clearance

Douglas Young Youth and Family Services is a children/youth outpatient program which provides services to theCentral/North Centralregions of San Diego County. We provide a range of services including crisis intervention, individual, family, and group therapy, medication support, rehabilitation, case management and drug and alcohol services while incorporating the family systems and wrap-around approach. We provide mental health services to children and youth with Medi-Cal as well as those who are uninsured and underinsured.

Services are provided in English and several threshold languages, using medical care, education, and family resources as appropriate. CRF staff members work together to meet the needs of San Diego Countys residents who are experiencing mental/behavioral health challenges.
