$62,000 - $67,000/year Reports to: Housing Director, Co-Executive Director Organization Description The Miss Major Alexander L. Lee TGIJP Black Trans Cultural Center is a team of transgender, gender-variant and intersex people, inside and outside of prisons, jails, detention centers, and other locked facilities creating a united family in the struggle for survival and freedom. We work in collaboration with others to forge a culture of resistance and resilience to strengthen us for the fight against human rights abuses, imprisonment, police violence, racism, poverty, and societal pressures. We seek to create a world rooted in self-determination, freedom of expression, and gender justice. Position Description Housing Case Managers at TGIJP are expected to provide comprehensive case management services to clients experiencing housing insecurity or homelessness. This role involves assessing client needs, developing housing plans, advocating for client rights, and connecting clients with appropriate resources and services. Housing Case Managers work closely with clients to empower them to achieve housing stability and improve their overall well-being. Primary Responsibilities 1.Client Assessment and Case Planning:
Conduct comprehensive assessments of clients' housing needs and barriers. Develop individualized housing plans in collaboration with clients. Set goals and action steps to address housing challenges and promote stability. 2.Resource Referral and Coordination:
Connect clients with community resources, social services, and support networks. Coordinate referrals for mental health services, substance abuse treatment, and other supportive programs. Help clients access financial assistance, food assistance, and other basic needs resources. 3.Crisis Intervention and Support:
Provide crisis intervention and emotional support to clients in housing crisis situations. Recognize early signs of emotional distress, agitation, or conflict Maintain a calm and respectful non-confrontational approach Collaborate with other team members and external partners to address immediate housing needs and ensure client safety. 4.Documentation and Reporting:
Maintain accurate and up-to-date client records and case notes. Complete required documentation for program reporting and compliance purposes. Ensure client confidentiality & privacy in accordance with agency policies & regulations. 5.Travel and Home Visits:
Entering client homes in a respectful, professional, and non-intrusive manner Must have the ability to travel to clients' place of residence Must be comfortable working in person with clients to assist with needs 5.Advocacy and Empowerment:
Advocate for clients' rights and interests in housing-related matters as needed. Empower clients to self-advocate and navigate housing systems independently. Promote client empowerment and self-sufficiency through skill-building and education. 6.Budgeting and Analytical Skills:
Ability to create and implement client budgets, assist clients in tracking spending and expenses Strong organizational and record-keeping skills 7.Confidentiality and Professionalism:
Knowledge of HIPAA standards and protected health information Ability to practice privacy, confidentiality, and record private information safely and securely Keep client/case manager privilege in retaining confidential informationQualifications:
Education:
Bachelor's degree in Social Work, Psychology, Counseling, or related field preferred.Experience:
Previous experience in case management, social services, or housing advocacy preferred.Skills:
Strong interpersonal and communication skills. Ability to build rapport and establish trusting relationships with diverse populations. Knowledge of housing resources, rental assistance programs, and housing laws/regulations. Ability to work collaboratively in a team environment and independently. Strong organizational and time management skills. Commitment to social justice, equity, and anti-oppressive practices. Proficiency in Google Suite and database management.Working Conditions:
This position may require occasional evening or weekend hours to accommodate client needs. Travel within the local community will be necessary for outreach and resource coordination. How toApply :
