Requirements & Qualifications:
Bachelor's degree preferred, or equivalent healthcare-related experience Valid driver's license and reliable personal vehicle required Ability to conduct local field visits for wellness checks and client support Strong documentation and organizational skills Experience handling sensitive or confidential information professionally Comfortable navigating emotionally challenging client and family situations Proficiency with Microsoft Office and document management systems Strong written and verbal communication skills Ability to work independently while collaborating closely with a small team Must Haves Experience coordinating care or services for seniors, vulnerable adults, or healthcare clients Ability to conduct wellness visits and assess client wellbeing, safety, and environmental concerns Experience communicating with healthcare providers, insurance companies, care facilities, or legal professionals Strong attention to detail with rigorous documentation and time tracking Comfortable managing sensitive conversations involving family conflict, aging, illness, or end-of-life transitions Ability to adapt quickly and problem-solve in unfamiliar or evolving client situations Reliable transportation and willingness to travel locally throughout the San Jose area Professional, calm demeanor with strong emotional intelligence Preferred Background & Skills Background in social work, elder care, care coordination, estate settlement, or legal administration Experience working in HIPAA-regulated or legally regulated environments Familiarity with fiduciary services, trust administration, or probate processes Experience with Clio Manage or other legal case management/billing systems Healthcare administration or patient advocacy experience Interest in pursuing Professional Fiduciary licensing or long-term growth within elder services Experience supporting both operational and client-facing responsibilities in a small business environment
