Enhanced Care Case Manager

AACI is seeking a compassionate Enhanced Care Management (ECM) Case Manager to support high-risk Medi-Cal members with complex medical and social needs. In this role, you will provide outreach, care coordination, and resource navigation to help clients improve their health, stability, and overall well-being. Key Responsibilities Conduct outreach and engagement with ECM-eligible community members Assess client needs and develop individualized care plans Coordinate care with healthcare providers and multidisciplinary teams Connect clients to housing, behavioral health, and community resources Maintain accurate documentation and case records Build relationships with community partners and service providers Conduct outreach in diverse community settings Qualifications Bachelor's degree in psychology, Social Work, Public Health, or related field (or equivalent experience) 1+ year of case management or related experience Knowledge of community-based resources and social services Strong communication, organizational, and problem-solving skills Ability to work with diverse communities and multidisciplinary teams Valid California driver's license and reliable transportation Preferred Bilingual in Spanish, Vietnamese, Mandarin, Cantonese, or Tagalog Your work here will directly support individuals facing complex challenges and help them build healthier, more stable lives.

Pay:

$25.00 - $28.00 per hour
