Job Description
Role Summary Mary's Path changes lives Two at a Time. Pregnant and parenting teenage moms in the foster care system are placed with us. These teen moms who have been abused/neglected/trafficked for sex find a safe place to heal from their trauma, get back on the path to finish high school, and learn to be loving parents. We aim to break the cycle of poverty and abuse and show these teen moms they can live lives of dignity, hope, and self-sufficiency. As a Case Manager, you play an essential role through supporting Mary's Path's clients and their families through coordination of care and support, advocacy, crisis intervention, skill-building, education and more. You will work collaboratively with the Clinical and Mental Health team, and report to the Clinical Supervisor. Role Responsibilities Assist in completing the intake study of the resident Conduct comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan Link residents to community resources, including housing, medical care, mental health services, vocational services, employment, and financial assistance Engage with clients as requested and as appropriate to build relationships and trust with clients to support therapeutic goals Provide brief individual intervention to address client behavioral health needs, primarily focused on building motivation for ongoing treatment, development and facilitation of the child and family team and linkage to community resources Develop Child and Family teams for individualized treatment planning and increasing natural support available to youth Coordinate case conferences with residents, consults with service team, monitors the progress of residents on case plan, assists residents to meet the agreed upon goals Provide crisis management and immediate therapeutic response as part of the treatment team to clients experiencing acute symptoms associated with their behavioral health issues Provide professional and culturally relevant support, advocacy, information, and referrals to program participants to guide them through a process of self-directed goal setting that builds on client strengths and talents Participate in the Program Team meetings, service reviews, program staff meetings, and other meetings when necessary Act as a liaison between residents and service providers to ensure care is tailored to their needs and to reduce barriers to services. Acts as advocate for residents before judicial, community, social service, and administrative bodies as needed Assist clients in developing and learning appropriate methods of advocating for themselves Assist residents in becoming "document-ready" (e.g., state ID, birth certificate). Achieve a broad understanding of the resources and services available on site and in the community for clients with acute behavioral health needs. This may include information such as benefit entitlement, drug and alcohol treatment programs, vocational training, legal services, USCIS and immigration services, health and mental health services, housing stabilization, community resources Maintain caseload of assigned clients Complete progress notes, Needs and Services Plans, CFT Action Plans, Special Incident Reports and other required paperwork in a timely manner Participate in individual and/or group supervision Other duties as assigned by the Clinical Supervisor Required Qualifications and Experience Bilingual English/Spanish fluency required Excellent communication skills, including written, verbal, and nonverbal communication Proficiency in Microsoft Office and Microsoft 365 applications, including Outlook, Word, Excel, Teams, and OneDrive Ability to work independently in the office and in the community while maintaining a high level of accountability and self-management Ability to work with a diverse population, build trust, demonstrate interpersonal savvy, and exercise cultural competency Experience working collaboratively and effectively as part of a multidisciplinary team Ability to manage conflict and complexity professionally Demonstrated strength in the following competencies: collaboration, empathy, resilience, situational adaptability, and valuing differences across diverse populations Must have and maintain a valid California driver's license, safe driving record, and reliable transportation Due to the nature of this position's duties, only women may hold this position Physical Requirements and Environmental Conditions The conditions and requirements below are representative of those required for an employee to successfully meet the essential requirements of the role. Medical, Dental, and Vision Insurance:
Coverage begins on the first day of the month following 30 days of employment. Employees may choose from available medical plan options, including Kaiser HMO and PPO options. Employee-only premiums for the Kaiser HMO plan are approximately $30 per month. Dependent coverage is available at employee cost. Health Reimbursement Arrangement (HRA): Employer-funded HRA support of up to $6,000 annually for employee-only medical coverage, or up to $12,000 annually for employees enrolled with dependents. HRA funds may be used for eligible deductible and out-of-pocket medical expenses in accordance with plan guidelines. 403(b)
Retirement Plan:
Eligible after 30 days of employment, with a 100% employer match on the first 6% of employee contributions. Employer contributions vest over time; employee contributions are always 100% vested. Paid Time Off (PTO): PTO begins accruing from the date of hire. Full-time employees working 40 hours per week accrue PTO at 2.46 hours per biweekly pay period, for 26 pay periods per year. PTO accrual is proportional based on hours worked.
Paid Sick Leave:
Employees receive 40 hours of paid sick leave per year, in accordance with California law.
Paid Holidays:
Full-time employees are eligible for six paid holidays annually.

Employer-paid $50,000 coverage at no cost to the employee.
