Job Description

Position Summary The Engagement Specialist, in
RESPONSE
performs outreach to individuals and families experiencing homelessness or mental health crises as part of a collaborative, city-based mobile mental health crisis response team. This position responds to calls dispatched through the Santa Rosa Police Department's 911 Call Center and engages both housed community members and unsheltered individuals who are not being served or are underserved by existing community service delivery systems. Services are provided in the field and at the in

office and include conducting mental health assessments, developing and implementing mental health safety plans, providing crisis intervention and linkage to resources, and maintaining accurate, timely, and clinically appropriate case notes. The Engagement Specialist documents services in SmartCare and other required data platforms in accordance with program and compliance standards. Essential Functions 1. Provide engagement services as a member of the in
RESPONSE
team and create client service engagement opportunities for homeless individuals in crisis situations. 2. Follow Engagement, Assessment, and Referrals (E.A.R.) procedure as defined in Catholic Charities of Northwest California (CCNWC) Outreach Services (OS) Policy and Procedures. 3. Facilitate and maintain community relationships to create referral pathways for program participants into CCNWC's programs as well as resources outside of the agency. 4. Assist with screening of potential program participants for the Homelessness Prevention and Rapid Rehousing programs. 5. Provide program materials and referral training sessions to community partners regarding shelter services, including outreach to motels, churches, and downtown merchants. 6. Provide support and guidance to participants as they transition from street to shelter. 7. Follow safety protocols for community street outreach. 8. Perform job functions in line with the policies and procedures guiding the work of this position, the program, and
CCNWC. 9.
Research and implement best practices in street outreach and assist participants in acquiring resources for acquiring housing. 10. Develop and manage a comprehensive outreach/ working relationship with all stakeholders, elected officials and staff, commercial and private community activists, and clients. 11. Divert or link individuals and families with the appropriate services, including Coordinated Entry, diversion, crisis response and intervention, shelter referrals, temporary housing, permanent housing, and Rapid Re-Housing. 12. Conduct mental health assessments, develop and implement mental health safety plans, provide crisis intervention with the individual/family the team is dispatched to for service. Provide linkage to resources within the community and referrals to HOST and the System Navigation team. 13. Maintain accurate, complete, and timely clinical records in accordance with Medi-Cal and agency requirements. All required documentation will be completed in SmartCare, following established policies and protocols. Participate in mandatory training related to SmartCare usage and Medi-Cal compliance. 14. Maintain accurate data collection and reporting across all other required data platforms, including all databases and any newly implemented systems or platforms. 15. Provide thorough and complete documentation of all required activities and engagements, per contract requirements, including Problem Intervention Response Plan (P.I.R.P.) case notes, and documentation of reasonable accommodation requests. 16. Serve on an agency or community service committee representing the program or the agency as assigned. 17. Attend mandatory supervision meetings to ensure accurate assessment, oversight, and data collection, including weekly meetings with the assigned supervisor or manager and monthly supervision meetings with the InRESPONSE Program Manager/Program Director to support compliance, program quality, and accurate documentation and data reporting. 18. Conduct outreach worker related duties independently and exercise sound professional and service intervention judgement, requiring minimal documentation errors in corrective actions. 19. Education, Experience, and Skills Required 1. High school diploma or High School Equivalency (HSE) and one of the following is required: Two years lived mental health experience, including supporting a friend or family member with a mental health disorder Two years of volunteering within the behavioral health system Two years of work experience in the behavioral health field Associates degree related to mental health 2. Understanding of causes of homelessness, experience working within social services, knowledge of Housing First, street outreach methods, and relevant community resources. 3. Completion of training and/or certification to be able to provide specialized services and increased performance as an outreach worker. 4. Ability to collaborate with law enforcement, EMS and crisis stabilization mental health clinicians in a crisis setting. 5. Obtain Non-Violent Crisis Intervention (CPI), CPR and First Aid certification within six months of transfer to this position or hire date. 6. Ability to work under pressure with a positive, solution-oriented approach. Experience in crisis intervention and problem solving with ability to diffuse an intense situation. 7. Ability to learn and apply various engagement techniques to a variety of situations. 8. Cooperative, friendly, and helpful attitude with clients and co-workers. Ability to work closely with other employees to ensure a positive, constructive environment within the program and throughout the agency. 9. Demonstrated computer literacy required, including proficiency with Microsoft Office 365 applications. Willingness to learn and apply new software and platforms as necessary for the work. 10. Strong written and verbal communication; organizational, conflict resolution and problem-solving skills. 11. Ability to communicate, orally and in writing, clearly and respectfully with clients and colleagues, individually, and in group settings. 12. Ability to learn, comprehend, and concentrate on essential job functions and carry our job functions with minimal guidance or reminders to successfully complete essential functions with timeliness and organization. 13. Ability to operate and understand basic radio codes under SRPD's dispatch protocols and to operate and manage the Mobile Communication Device (MCD) and portable radios to maintain real-time communication with Santa Rosa Police Department to dispatch during crisis response events to ensure accurate and timely updates to support coordinated interventions. 14.
