Job Description
The Village of Hope Case Manager assist students towards self-sufficiency through accountability, mentoring, instruction, and coordination of services. Serves as the case manager for survivors of sex trafficking in a partnership to provide Biblical counseling, comprehensive services, and support; ensures comprehensive social services are delivered promptly, including but not limited to: referrals to medical, mental health, dental, immigration/legal providers; access to appropriate benefits, housing, childcare, supportive counseling/socialization, literacy, criminal justice system victim advocates; family reunification and coordination with life skill mentors and interpreters. This is a full-time position. Schedule to be determined. Some of the Job Duties are: Essential Job Duties with optional subcategories (Please add time spent in percentage for each job duty): 1. Follow and adhere to all policies and procedures while assisting studentsthrough biblical counsel, listening, prayer, and completing their Individualized Plan, which may include resolving scheduling, relational, and programmatic conflicts, family reunification, and parenting programs. 2. Maintain up to date records and keep all student paperwork orderly and adequately filed, protect student privacy and maintain confidentiality at all times 3. Participate fully in implementing program changes and directives from management 4. Assure quality and coordination of services, assess and monitor the progress of individual students, challenges, record keeping, and chart audits 5. Teach new life skills and coping skills so that students can replace bad behavior with good behavior, and hold weekly group meetings to help students overcome their life challenges with peer support. 6. Meets weekly with each assigned student (up to 30 sophomores) to assess progress and provide case management counsel. 7. Provide and follow up on appropriate disciplinary actions, including dismissal from the program as needed. 8. Meet and communicate weekly with the direct supervisor. 9. Maintain a clean, safe, and unobstructed workplace. 10. Conduct intakes to determine the continuance of program enrollment based on established guidelines and requirements 11. Provides direct case management for all eligible survivors referred to the program and assessment, as necessary. Serves as an on-call worker for survivor identification and outreach and meets regularly with survivor students to discuss and evaluate their progress to ensure that services are delivered to clients using a safe client-centered, Biblical, strength-based approach. 12. Assist survivors of human trafficking in obtaining allowed medical care, counseling, legal services, and long-term housing. 13. Assumes responsibility for accomplishing program goals, which include providing targeted and culturally-sensitive services to survivors, enhancing the ability of professionals in the community to respond appropriately, increasing community collaboration and coordination, and increasing the number of survivors identified and served in the area. 14. Coordinates with key partnerships and core volunteers 15. Ensure timely development of protocols to guide the local response to cases of trafficking. 16. Facilitate group and individual counseling to trafficked persons using evidence-based practices, such as Cognitive Behavioral Therapy, Seeking Safety, Trauma Resiliency Empowerment Model, etc. 17. Refers clients to appropriate programs or other agencies based on clients' needs and following their program plan. 18. Maintain confidentiality of all records, reports, and other materials relating to survivors of trafficking 19. Completes discharge plans of clients exiting the program; transfers file contents to non-active file; prepares card for a follow-up mailing
Job Skills/Qualifications:
Bachelor's Degree or counseling credentials and experience in a related field such as in a social work/counseling environment or within a ministry setting. Knowledge and experience working with men, women and children in crisis. Strong working knowledge of the Bible. Strong verbal and written communication, organizational and administrative skills. Christian Association of Drug and Alcohol Counselor (CADAC) or Registered Addictions Specialist (RAS) certification desired Part II -Job Requirements Education:

Bachelor's Degree or counseling credential in a related field such as in a social work/counseling environment or within a ministry setting and a minimum of 1-year related work experience providing case management to trafficking survivors, victims of crime, domestic violence, immigrant communities, or similar population is preferred.
