Case managers support residents of the Fraternity House, Inc.'s housing programs work toward accomplishing goals related to their health autonomy, independence, housing security, and overall wellbeing. The case manager is responsible for all aspects of managing resident care, including, but not limited to: case plan, care management, assuring residents receive quality care and are connected to services required for optimal outcomes. POSITION RESPONSIBILITIES
1. Conduct initial and ongoing comprehensive assessments of residents' needs, including (but not limited to): housing, social, financial, and health domains. 2. Enhance services available by identifying and facilitating services available through external agencies and community partners. 3. Aid residents in receiving assistance-based services, such as healthcare & mental health needs, nutrition assistance programs, Social Security-related services, and more. 4. Maintain occupancy rates for the program by maintaining a waitlist, conducting intake interviews, and dispersing, facilitating, and collecting required intake documentation for move-ins. 5. Maintain accurate, up to date, and comprehensive resident files to include case notes, income determinations, and housing and case plans 6. Provide support and resources for residents enduring crisis, possibly during after-hours or at unexpected times. 7. Assist residents in complying with program requirements to maintain eligibility and a general good-standing status with FHI. 8. Other duties as assigned QUALIFICATIONS
1. Previous experience as a case manager preferred. 2. Knowledge of alcohol and substance abuse, and mental health issues is preferred. 3. Knowledge of HIV/AIDS
is required. 4. Sensitivity to working with culturally diverse populations and ability to maintain confidentiality. 5. Excellent verbal & written communication, interpersonal, and oral presentation skills 6. Computer proficiency a must. 7. Bilingual / Spanish
a plus. 8. A degree in social work, psychology, human services, or a related field is preferred. WORKING ENVIORNMENT
This position requires a combination of standing and sitting, as well as frequent lifting of up to 20 pounds, turning and bending.
