Manage a specialized caseload of 15-20 justice-involved individuals, providing continuous, high-touch support across the transition from a correctional setting to community stabilizationStabilization & Advocacy:
Drive efforts to increase client success by proactively securing stable housing, facilitating access to substance use recovery programs, and managing critical mental health linkagesBilingual Clinical & Cultural Navigation:
Serve as a specialized bilingual liaison, providing culturally responsive advocacy and case planning in both English and Spanish to ensure equitable access to services and eliminate language barriersSystems Coordination:
Act as a primary point of contact for a multidisciplinary network, including high-level coordination with Mental Health Partners (MHP), local housing authorities, and other partnering agencies—both internal and externalData Integrity & Reporting:
Maintain rigorous, professional documentation and track client progress in alignment with departmental standards to ensure program accountability and outcome measurementIn-Custody Coordination:
Conducts one-on-one case planning sessions within the Boulder County Jail to prepare clients for releaseMultidisciplinary Collaboration:
Participates in team meetings with jail medical staff, security, and transition teams to ensure a seamless continuum of careField-Based Outreach:
Performs direct outreach in the community to engage with unhoused clients and monitor progress toward stabilization goals. This includes, but is not limited to, work in parks, shelters, and private residencesResource Advocacy & Housing Support:
Facilitates client access to essential services by identifying resources, supporting engagement with Medication-Assisted Treatment (MAT) clinics, and coordinating with housing authorities to secure stable placementClinical Documentation:
Maintains meticulous electronic records and performs data entry to effectively track client outcomes and milestonesStrategic Liaison:
Acts as a bridge between clients and various community resources, ensuring referrals are completed and that the client is successfully linked to necessary behavioral health services Performs related work, as required May be reassigned during emergency situationsPLEASE NOTE

Boulder County is looking for well qualified candidates to fill our positions. Any combination of relevant education and experience is encouraged. In this position, we are looking for a minimum of 6 years of combined education and experience in human services, case management, sociology, criminal justice, psychology, or a related field . Applicants must demonstrate English and Spanish fluency at the time of the pre-employment screening process. Spanish language proficiency will be determined through standardized verbal/verbal and written demonstration. (tier 3 written and verbal) Additional compensation will be provided based on the use of bilingual skills.PREFERRED QUALIFICATIONS
Ability to pass a criminal background investigation and fingerprinting (mandatory for jail and Community Justice Services clearance).KNOWLEDGE, SKILLS, & ABILITIES
