Job Description

Starting hiring salary - $70,000 to $74,200 annually YouthLink - Resource and Assessment Center is a newly established nonprofit 501(c)(3) serving Adams County, CO, dedicated to strengthening families and supporting youth through early intervention, assessment, and coordinated access to community resources. YouthLink accepts referrals from law enforcement when concerning behavior is identified and provides timely assessment, trauma-informed care coordination, and connections to culturally responsive supports aligned with each young person's strengths and family context. Serving youth ages 12-17, YouthLink delivers early, preventative support before challenges escalate into school suspension, court involvement, or emergency response.

Preventing and diverting youth from the child welfare and justice systems Identifying underlying issues contributing to behavior or conflict Partnering with young people and their families to connect them with individualized,culturally responsive supports
DEFINITION
This position provides programmatic leadership and operational oversight and partnership representation for YouthLink, a nonprofit 501(c)(3) serving young people and their families in partnership with agencies and community stakeholders across Adams County. In collaboration with the Executive Director, drives organizational mission, vision, and strategy. The Case Manager works closely with the Program Director and Executive Director to advance YouthLink's mission and priorities. This position supports the program team, interns, and volunteers.
ESSENTIAL RESPONSIBILITIES AND DUTIES
1.Case Management, Coordination, and Oversight Manage incoming referrals and assess immediate needs, risk factors, and servicepriorities. Develop individualized, strengths-based support plans in partnership with youth and families. Identify, vet, and connect youth to appropriate community-based services and supports. Track case progress, ensure timely follow-through, and remove barriers to service access. Coordinate with schools, including oversight of school engagement and summerprogramming support. Maintain consistent communication with youth, families, and service providers to ensure alignment and accountability. 2. Data Monitoring and Reporting Maintain accurate, timely, and compliant case records in accordance with organizational and funding requirements. Collect, track, and disaggregate key performance metrics, including attendance, behavior, service contacts, and outcomes. Prepare brief case updates, summaries, and dashboards to support program evaluation and continuous improvement. Ensure documentation reflects trauma-informed and culturally responsive practices. 3. Cross-Agency Collaboration and Family Engagement Facilitate warm handoffs and coordinated service delivery with schools, law enforcement, behavioral health providers, and community partners. Convene and participate in case coordination meetings and multidisciplinary team huddles. Build and maintain strong, trust-based relationships with youth and caregivers. Communicate regularly with families to provide updates, support engagement, and address concerns. Support safety planning and crisis response in collaboration with families and partners. Apply culturally responsive and trauma-informed approaches in all interactions and service delivery. 4. Program Implementation and Service Quality Ensure services are delivered in alignment with YouthLink's mission, model, and quality standards. Monitor service timelines and responsiveness to ensure early intervention goals are met. Identify service gaps and escalate concerns or barriers to the Program Director. Participate in team meetings, case reviews, and continuous improvement activities.
QUALIFICATIONS
Knowledge of:
Trauma-informed care, culturally responsive practices, and positive youth development. Youth intervention, diversion, and case management approaches. Community resources, school systems, and behavioral health services (Adams Countypreferred). Data tracking, documentation standards, and basic reporting practices.
Ability to:
Work nights and weekends as needed (anticipated schedule being Wednesday-Saturday or Sunday-Wednesday) Build trust and rapport with youth and families from diverse backgrounds. Manage multiple cases and competing priorities in a fast-paced environment. Communicate clearly and effectively with partners, families, and team members. Navigate complex systems and coordinate across multiple service providers. Demonstrate strong organizational, problem-solving, and time management skills. Maintain professionalism, confidentiality, and sound judgment in sensitive situations. Work independently while contributing to a collaborative team environment.
EDUCATION AND EXPERIENCE
Bachelor's degree in social work, criminal justice, psychology, human services, or a related field-Preferred. 2-5 years of experience in case management, youth services, school-based support, orcommunity-based programs - Preferred. Equivalent combination of education and relevant experience will be considered.
PHYSICAL REQUIREMENTS
While performing the duties of this position, the employee is regularly required to sit, stand, walk, talk, listen, drive to meetings, operate a computer, and lift objects to 10 pounds.
